Game Registration Process:
1. Copy of Team Roster with guest players and all jersey numbers written in.
2. Player cards. If your player is coming without a card, you must have a birth certificate and proof that your club carries liability insurance for this player. A card proves that your player is covered by your state association.
3. Medical release forms...do not have to be notarized.
4. Coach's responsibility statement ....which can be found int the acceptance link above.

Registration fees for 11v11 is $575.00 and 8v8 is $500.00.
U12 will have 11v11 and 8v8 divisions.
